If you have an HMO insurance plan or your insurance requires a referral, please contact your Primary Care Physician (PCP) to request that a referral be sent to our office. Patients are responsible for ensuring that a valid referral is on file prior to scheduling an appointment. We must receive the required referral before an appointment can be scheduled.
